Review of cryptogenic fibrosing alveolitis, including current treatment guidelines.

چکیده
Cryptogenic fibrosing alveolitis (CFA), known as idiopathic pulmonary fibrosis in the USA, is characterised by inflammation and fibrosis of the alveoli and interstitium of the lungs, favouring the subpleural and basal regions. By definition the diagnosis demands that all known causes of pulmonary fibrosis have been excluded. It is now clear that the CFA population is comprised of a heterogeneous group, with varying clinical, radiological and histological features, which in turn may be used to predict response to treatment and prognosis. The British Thoracic Society has recently produced guidelines on the investigation and management of diVuse parenchymal lung disease, including CFA.

Hypothesis: exposure to solvents may cause fibrosing alveolitis.
The incidence of fibrosing alveolitis, which can be caused by many agents, is increasing. In a proportion of patients, the aetiology is unknown (cryptogenic fibrosing alveolitis), but it has been suggested that environmental factors may play an important, but unrecognized, role in its pathogenesis. A review of the literature suggests that there may be a link between exposure to solvents, widesp...

OBJECTIVE To determine the pattern of mortality ascribed to cryptogenic fibrosing alveolitis and to identify factors that might be important in the aetiology of the disease; and to assess the validity of death certification of the disease. DESIGN A retrospective examination of mortality ascribed to cryptogenic fibrosing alveolitis in England and Wales between 1979 and 1988 with analysis, by m...
متن کاملHigh resolution computed tomographic assessment of asbestosis and cryptogenic fibrosing alveolitis: a comparative study.
BACKGROUND The aim of this study was to compare the distribution and configuration of lung opacities in patients with cryptogenic fibrosing alveolitis and asbestosis by high resolution computed tomography. METHODS Eighteen patients with cryptogenic fibrosing alveolitis and 24 with asbestosis were studied. Two independent observers assessed the type and distributions of opacities in the upper,...
متن کاملAccuracy of diagnostic coding of hospital admissions for cryptogenic fibrosing alveolitis.
To determine the accuracy of diagnostic coding of cryptogenic fibrosing alveolitis, the case notes of 166 admissions to four hospitals were reviewed. These consisted of all admissions that had been coded as "idiopathic fibrosing alveolitis" (ICD code 516.3: 97 admissions) or as "postinflammatory pulmonary fibrosis" (ICD code 515.9: 69 admissions). Of 88 available records of admissions coded as ...
متن کاملAerosolised ribavirin in patients with advanced cryptogenic fibrosing alveolitis: a pilot study.
BACKGROUND A report has recently been published concerning a patient with a cryptogenic fibrosing alveolitis who showed a striking improvement after being treated with the antiviral drug ribavirin (tribavirin, Virazid). The objective of this study was to further evaluate, in an open trial, the efficacy of rivabirin in cryptogenic fibrosing alveolitis. METHODS Ten patients (eight women) with a...
